Funding Opportunity Title: Funding for Medical Travel Expenses
Description of Funding Opportunity: This funding opportunity was created to help supplement expenses associated with paying for travel related to for medical care. The funding can be used for:
- airline, train, or bus ticket for medical travel
- gasoline and/or rental car fees
- hotel expenses related to medical travel
If you need funding for something that is not listed above, you can still submit your request and our Board of Directors will review the request for approval.
Special Note: This funding cannot be used for travel associated with a cosmetic procedure that is being performed to enhance or improve aesthetic appearance. However, this funding can be used for travel for reconstructive procedures that are being performed to address a physical impairment, medical impairment, or chronic pain. We do support people’s right to change their appearance, but we have to restrict our funding to people that are in dire need of funding for serious medical needs.
- We Would Approve- Rhinoplasty patient who needs the surgery to correct a structural issue that severely restricts their breathing
- We Would Deny– Rhinoplasty patient who wants the surgery to change the way their nose looks for appearance reasons.
Entry Period and Winner Announcements: This funding opportunity will open at the beginning of each month and close at the end of each month, unless otherwise noted on this page. Winners will be announced at the end of the following month. (Example: For entries submitted between January 1- January 31, a winner will be notified by the end of February.)
Amount of Funding: up to $200
Eligibility Requirements:
- Applicants must be at least 18 years of age; the beneficiary of the grant can be any age
- This funding cannot be used to travel for cosmetic procedures, unless the cosmetic procedure
- Applicants must complete the funding application at the bottom of this page.
- Applicant must complete a short explanation that explains why they are requesting this funding. (100 to 200 words)
- Applicants must include proof of expenses. Examples include receipt or bill

Explanation Requirements: Using 100 to 200 words, please explain to us why you are requesting this funding. As you are writing, you should include the following:
- Tell us all about the you and/or the person receiving the medical care. Where do you live and where do you have to travel for your medical care?
- What will the funds be used for? Be specific about how this money will help with medical care.
- Tell us specifics about any fees/costs that you have received or will receive
Upload a file: You must send us proof of what you intend to use the funds for along with proof of the medical procedure date. You can submit a receipt for a travel expense, screenshot of a travel expense you intend to pay for, a bill/invoice from a medical institution or business, or similar types of documentation. You will only be able to upload one file so combine your proof into one document.
The only personal information that can be shown in your document is your name. You should remove all other personal and private information from the file you upload! For your safety, cover up or black out any private information such as addresses, birthdates, social security numbers, account numbers, etc.
